APPLICANT REQUESTS THAT:

His DD Form 214, Report of Separation From Active Duty, be corrected to reflect the following:

      a.  Primary Specialty Number and Title; 9351 , Flight Surgeon instead of 9326, General Duty Physician. 
      
      b.  Related Civilian Occupation and D.O.T. Number; Aerospace Medicine Physician instead of 070.108, General Practitioner.  
      
      c.  Course 50BY, 9351 Aerospace Medicine Primary Course. (administratively corrected).  
      
      d.  Certificate of Completion for USAF Survival and Special Training School, dated 19 Sep 1975.  
      
      e.  Certificate of Appreciation, dated 4 Feb 1976.  
      
      f.  Letter of Appreciation, dated 20 Apr 1976.


APPLICANT CONTENDS THAT:

He requested corrections to his DD Form 214 on 24 Aug 1990; however, it was never accomplished.  

The applicant’s complete submission, with attachments, is at Exhibit A.


STATEMENT OF FACTS:

According to the applicant’s military personnel records, he was honorably released from extended active duty and transferred to the Air Force Reserve on 2 Jul 1977.  He was credited with three years total active service for this period.  

On 3 Feb 1977, the applicant’s AF Form 707, Officer Effectiveness Report (OER), rendered for the period 1 May 1976 thru 25 Jan 1977, reflects his duty title as Hospital Aerospace Medicine Physician.  

On 7 Mar 2014, AFPC/DPSIT notified the applicant that he had completed the Aerospace Medicine Primary Course on 19 Sep 1975; therefore, the training corrective actions he requested had been resolved through pertinent administrative procedures.  

On 7 Jul 2014, AFPC/DPSID was able to verify the applicant’s entitlement to the National Defense Service Medal (NDSM) and will correct his records administratively.  

The remaining relevant facts pertaining to this application are described in the letters prepared by the Air Force offices of primary responsibility (OPR), which are attached at Exhibits C and D.  


AIR FORCE EVALUATION:

AFPC/DPSIC recommends approval to correct the applicant’s DD Form 214 to reflect his Air Force Specialty Code (AFSC) as 9351, Aerospace Medicine Physician with no corresponding D.O.T. number and 9326, General Duty Physician (D.O.T. number 070.108 General Practitioner) as the secondary specialty.  Currently, the  applicant’s DD Form 214 specialty information reads, “9326, General Duty Physician with D.O.T. number 070.108, General Practitioner, and 9411 Surgeon with D.O.T. number 070.101, as the primary and secondary specialties, respectively.  However, there is no evidence the applicant performed duty in AFSC 9411.  The correct title of AFSC 9351 is Aerospace Medicine Physician as there is no specific AFSC for Flight Surgeon.  The applicant submitted various documents which referenced completion of the Aerospace Medicine Primary Course and aeronautical rating of Flight Surgeon.  The fact that the applicant’s record supports duty in AFSC 9351, following duty in AFSC 9326, his correct primary AFSC should be AFSC 9351.  His records indicate he met the requirements for award of AFSC 9351, had the aeronautical rating of Flight Surgeon, and subsequently served in that AFSC per his officer effectiveness reports.  In summary, the applicant’s DD Form 214 incorrectly identifies his Primary Specialty as 9326, General Duty Physician and secondary as 9411, Surgeon.  It should read 9351, Aerospace Medicine Physician (no corresponding D.O.T. number in block 16b) as the primary specialty and 9326, General Duty Physician (D.O.T. number 070.108 General Practitioner) as the secondary specialty.

AFPC/DPSID recommends denial of the applicant’s request to update his DD Form 214 to reflect his Certificate of Appreciation, dated 4 Feb 1976 and Letter of Appreciation, dated 20 Apr 1976.  These documents are not updated to the DD Form 214.  To grant relief would be contrary to the eligibility criteria established by DoDM 13448.33.

APPLICANT'S REVIEW OF AIR FORCE EVALUATION:

Copies of the Air Force evaluations were forwarded to the applicant on 8 September 2014 for review and comment within 30 days.  As of this date, no response has been received by this office (Exhibit E).


THE BOARD CONCLUDES THAT:

1.  The applicant has exhausted all remedies provided by existing law or regulations.

2.  The application was timely filed.  

3.  Insufficient relevant evidence has been presented to demonstrate the existence of an error or injustice with respect to the applicant’s request for his certificate of appreciation and letter of appreciation be reflected on his DD Form 214.  In this respect, we note the comments by AFPC/DPSID that these documents are not updated on the DD Form 214.  Additionally, we took note that the applicant has requested that his DD Form 214 be corrected to reflect his Certificate of Completion for the USAF Survival and Special Training School, however, he has not provided any evidence that he attended this training.  In view of this and in the absence of evidence to the contrary, there is no basis to recommend granting the requested relief.   

4.  Notwithstanding the above, sufficient relevant evidence has been presented to warrant some relief.   With respect to the applicant’s request to correct his primary and secondary Air Force Specialty on his DD Form 214, we agree with the opinion and recommendation of AFPC/DPSIC and adopt its rationale as the basis for our conclusion that the applicant’s record be corrected to the extent indicated below.  


THE BOARD RECOMMENDS THAT:

The pertinent military records of the Department of the Air Force relating to the APPLICANT be corrected to show that:

		a.  His DD Form 214, Report of Separation From Active Duty, effective 2 July 1977, Block 16a. Primary Specialty Number and Title, reflect 9351 Aerospace Medicine Physician instead of 9326 General Duty Physician.  

		b.  Block 16b. Related Civilian Occupation and D.O.T. Number, reflect no corresponding D.O.T. number instead of 070.108 General Practitioner.  

		c.  Block 17a. Secondary Specialty Number and Title, reflect 9326 General Duty Physician instead of 9411 Surgeon.  

		d.  Block 17b. Related Civilian Occupation and D.O.T. Number, 070.108 General Practitioner instead of 070.101 Surgeon.
